两者都有小心，当心的意思，但 be careful with 的宾语可以是人，be careful about 一般只是事物。供参考
其实没啥区别 前者是指让别人小心做什么事 后者是指小心某事
一般情况下be careful with doing sth be careful about sth.
1.be careful + about/of+ 名词 注意...的
eg.She is careful about/of her food. =She is careful about/of what she eats. 她很注意饮食。（后面界wh- 从句时，多省略介词）
2.be careful + with + 名词 注意处理...
eg.Be careful with the vase; it's valuable. 那花瓶很贵，要小心处理.
